6. By ordering a product, you give Elite MD the permission to charge your card in the amount due for the product, shipping, handling, and tax.

PURCHASING PROCEDURES AND GIFT CARDS FROM OUR ONLINE STORE
• All treatments are performed by a team of experts.
• All treatments require a consultation to confirm appropriate candidacy. If not deemed an appropriate candidate, funds may be used toward other purchases.
• Due to high demand, we may have limited availabilities.
• Gift cards may only be used for cosmetic dermatology services only.
• Injectables performed by a board certified Nurse Practitioner.
• They may NOT be used for plastic surgery services.
• They may NOT be used for online purchases.
• Elite MD is not responsible for lost gift cards.
• Gift cards must be used within 6 months of purchase.
